
A
Graphical Chart is a print out of a patient's data for one visit. The patient's
chart number, name, examiner, date of visit, and the date of the reference
visit are printed at the top. A PSR chart is also printed. Along the right
side of the chart there is a Diagnosis box and a Legend. The Diagnosis box
is available for you to check off the level of Gingivitis or Periodontitis.
The Legend lists the symbols for changes in Pocket Depth and indicators for Pocket
Depth, Recession, Minimal Attached Gingiva,
No Attached Gingiva, Bleeding, Suppuration,
Plaque, and Furcations.
The teeth are displayed in four rows from the right side to the left. The
facial, mandibular view is on top with the lingual view directly under it.
Then the lingual, mandibular view is displayed with the facial view at the
bottom. The tooth numbers are listed between the lingual and facial views
for each dental arch.
This chart can be printed on a preprinted form or plain paper. On the preprinted form, missing teeth are shaded over. On plain paper, missing teeth are not printed.
The new graphical chart has three minor changes. The first change you will notice is that any patient with medical alerts posted on the personal page will have a bright red cross and 'Medical Alert!' warning printed on the chart. For a more descriptive analysis you must view the information stored in the personal page. Plaque data printed in the summary box printed at the bottom of the chart will contain additional percentages (ex. 26% molar, 13% interproximal, 12% facial, 13% lingual). Symbols are now been used to represent tooth mobility. You will now see I, II, III in place of the 1, 2, 3 previously used. This is designed to make the chart easier to read and less likely to confuse the mobility numbers with other numbers on the chart.

Pocket Depth and Recession can be displayed three ways: A.L. (Attachment Level) line, Depth Bars, or both.
Only rounded recession values
are printed. In the case of one site recession, it will be located above the
facial, maxillary view and below the facial, mandibular view. If there were two
or six site readings, recession will also be listed below the lingual,
maxillary arch and above the lingual, mandibular arch. Minimal attached gingiva
is marked by
and no
attached gingiva is marked by
. These symbols are printed near the recession
values.
Only rounded Pocket Depth values
are printed. Normal levels are black; warning and danger
levels are red. Values for pocket depth are listed above the facial,
maxillary and lingual, mandibular views and below the lingual, maxillary and
facial, mandibular views. Arrows show changes in Pocket Depth from the reference visit. If the probing depth is one to two
millimeters deeper than the previous value for the same site, a
will appear. If it is two or more millimeters deeper,
will appear. If there is a one to two millimeter improvement
from the previous visit, a
will appear. If there is a two or more
millimeter improvement,
will appear.

The degree of mobility (0, I, II, III) will be listed below the lingual, maxillary view and above the lingual, mandibular view. Normal levels are black; warning and danger levels are red.
There is a complete summary of the data below the chart. It lists the number of teeth with the number and percentage of sites that have pocket depths greater than the warning level. It will then tell you the number and percentage of sites that deepened and improved by one millimeter or more. The number and percentage of buccal and lingual sites where bleeding occurred is listed. The number of sites where recession was found and the number of those sites that were greater than the warning level are also listed. The number of sites that had a furcation and some degree of mobility are displayed. The total number of sites, inter-proximal sites, lingual sites, and buccal sites where plaque occurred are listed last.
A plaque chart is located in the lower right corner. The plaque chart is printed as the patient would see the mouth in a mirror. Note that this is opposite the main chart that is shown as an examiner sees the mouth while facing the patient.
The warning symbol
will appear on the upper left of the chart if the patient has any Medical
Alerts stored in the database. This information is not printed on the
charts. Refer to the Personal Page for the patient to review the medical
information provided.
