
Numerical Reports show the same information as the Rectangular
Periodontal Chart . The data is listed in a column format. Recession and
probing Pocket Depth data are shown without rounding to the nearest 0.2 mm.The
chart number, patient's name, examiner's name, and examination date are at
the top of the page. The tests performed are marked by a red X. Data for each
tooth is listed in a separate, horizontal row, with the tooth number to the
left. Pocket Depth, Bleeding, Suppuration, Furcation, and Plaque are recorded
on a per site basis. If Recession is recorded
at two or six sites, it will also be printed on the site location. The data
for teeth numbered one to eight and twenty-five to thirty-two are listed from
Distal Buccal (DB), Buccal (B), Mesial Buccal (MB), Mesial Lingual (ML), Lingual
(L), to Distal Lingual (DL). The sites for teeth nine to twenty-four are listed
from Mesial Buccal (MB), Buccal (B), Distal Buccal (DB), Distal Lingual (DL),
Lingual (L), to Mesial Lingual (ML).
At each site, Pocket Depth is listed farthest to the left. Normal levels appear black, while warning and danger levels appear red.
If Bleeding occurred at the site, a red "B" will be printed to the right of the Pocket Depth values.
Suppuration is represented next by a black "S".
A furcation is recorded as F=(0-3). Values 1-3 appear red.
Plaque is listed farthest right as a pink "P".
Mobility is listed as M=(0, I, II, III). Values I, II, III appear red.
If recession was recorded on a one site basis, it will be displayed after mobility. If it was recorded on a two or six site basis, it will be displayed after plaque in the per site region. Recession appears as R=<#> where <#> refers to a 1-decimal place number. Normal levels appear black, while warning and danger levels appear red.
Plaque Index Scores and a Probing Summary are on the bottom of the page. The Plaque Index lists the percentage of plaque on all, molar, inter-proximal, buccal, and lingual surfaces. The Probing Summary lists the number of teeth present, the number of Bleeding sites, and the number of Suppuration sites. It also lists the number of all, moderate, and severe sites where Pocket Depth is progressing deeper by at least one millimeter. The number of Furcation sites and teeth where some Mobility occurs are also recorded.
