Rectangular Periodontal Chart
Rectangular Periodontal Charts represent the teeth
with rectangles. The patient's chart number, name, examiner, and examination date are
listed at the top. The tests performed and displayed on this chart are marked with a red
X. The date of the reference visit is listed next. This chart is set up from right to
left, looking into the mouth. The facial, maxillary view is shown in the first row with
the lingual view directly under it. Then the lingual, mandibular view is drawn with the
facial view at the bottom. The symbols for change in Pocket Depth,
minimal attached gingiva, and no attached gingiva used in the Graphical
Chart are also used for this chart.
Recession values are located
above the facial, maxillary view and below the facial, mandibular view. Values rounded to
the nearest whole number are listed. Black numbers represent normal values and red numbers
are used for warning and danger values. The symbol for minimal
attached gingiva is

Plaque is listed first for each row. Any site where plaque is recorded is represented as a pink "P".
Suppuration is then listed under plaque. Any site where suppuration occurs is marked by a black "S".
Bleeding is then listed. Any site where bleeding occur is marked by a red "B".
Next, Furcations are listed. Normal levels appear black, while warning and danger levels appear red.
Pocket Depth in millimeters is listed last. Values rounded
to the nearest whole number are displayed. Normal values are listed in black, while
warning and danger levels appear red. Arrows below these values show changes in Pocket
Depth from the reference visit. If the probing depth is one to
two millimeters deeper than the previous value for the same site, a
will
appear. If it is two millimeters or more deeper,
will appear. If there is a one to
two millimeter improvement from the previous visit, a
will appear. If there is a two or
more millimeter improvement,
will appear.
The warning level is listed under the diagram.
A Plaque Index and a Probing Summary are provided at the bottom of the page. The Plaque Index lists the percentages of surfaces with plaque that were found on all, molar, inter-proximal, buccal, and lingual surfaces of the teeth. The Probing Summary gives an overall description of the data. It lists the number of teeth in the mouth, the number of bleeding sites, and the number of suppurating sites. The number of all, moderate, and severe sites where pocket depth increased by more than one millimeter is displayed. The number of furcation sites and teeth that have some mobility are also listed.
